You can fill in the details using Word and then print off page 1.
Don't forget to check the box at the top which says that an HOCF has been completed.
Old format HOOFs will be accepted until the end of October 2009
Patients receiving oxygen for the first time will also need to complete a Home Oxygen Consent Form (HOCF) to give consent for the transfer of their personal details (eg name, address etc) to the Home Oxygen Service Supplier. You can download this form here:
