The Clinical Navigation service can offer the following alternatives to acute medical or surgical admission.
- DVT (check bloods and assess patient, if D-dimer positive will refer to DVT clinic on same day or start low molecular weight heparin overnight)
- reversal of excessive oral anticoagulation (check INR, give vitamin K)
- check abnormal blood results (take fresh bloods and take results, can directly admit when results available on GP request)
- possible malaria (check urgent blood count and film)
- CXR for pneumonia (report within one hour to enable GP to decide on home treatment)
- cellulitis (assess severity and treatment, including cannulate and administer IV antibiotics in NUPCAC)
- ECGs (faxed to GP)
- TIA Clinic (arrange same/next day clinic appointment)
- stroke beds (arrange direct admission)
- rheumatology clinic
- diabetic / diabetic foot clinic
- intermediate care / emergency home care / Red Cross CICS
- Breast Clinic (direct access for abscesses)
