Crisis Response Team

Prevent admission to hospital or a care home

The Crisis Response Team have capacity today.

Is your patient at imminent risk of being admitted to hospital or a care home due to an age-related illness or some breakdown in support or health and social care needs?

Could this admission be prevented if an assessment took place within four hours by a multi-professional clinical team, and up to four times a day health and social support calls were organised to meet the patient's presenting needs?

For example:
  • does the patient need emergency home care because they are not coping or their carer has become unwell / admitted to hospital?
  • does the patient need a piece of equipment to enable them to mobilise or transfer safely?
  • would medication prompts for a UTI or chest infection enable to patient to stay at home?
  • would support with social care needs and daily tasks such as washing and dressing avoid an admission?
