Monitoring thyroid function for replacement therapy

The 2006 UK guidelines published by the British Thyroid Foundation state that:
in determining the optimal dose of thyroxine, the biochemical target is a TSH result that is detectable, not elevated, and preferably within the reference range.
The risk of over suppression of TSH is an increased risk of atrial fibrillation and osteoporosis.

A TSH result of less than 0.1 should be considered as undetectable and consideration should be given to reducing the dose of levothyroxine.

However, TSH should be fully suppressed in patients who have had thyroid cancer.
